Volledige digitale data-integratie
CBCT/DICOM, IOS/STL, beetregistraties, foto’s en, waar geïndiceerd, gezichts- of prothesescans worden samengebracht in één digitale case voor de implant surgical guide. Zo wordt de communicatie tussen de praktijk en het laboratorium duidelijker en efficiënter.

Guides selected drills or stages of the osteotomy sequence. It offers more control than a pilot guide while allowing the clinician to complete other steps according to access and preference.
Fully Guided Implant Surgery
Controls the planned osteotomy sequence and, when supported by the implant system, implant insertion. For guided surgery dental implants, system-specific sleeves, drills, keys, offsets and protocols must be confirmed before production.
Stackable Surgical Guides
Uses modular, pin-stabilised components that share one reference and can be removed and repositioned during full-arch implant surgery like in All-on-X cases. Depending on the case, the stackable guide systems may include positioning, bone-reduction, implant-placement and prosthetic-conversion components.

A CAD resin surgical guide is digitally designed and 3D printed for efficient, patient-specific production. It suits many tooth-supported, mucosa-supported and selected bone-supported cases, subject to guide thickness, sleeve position, access and validation.
Metal Implant Surgical Guide
A metal surgical guide offers a rigid, low-profile option where access, stability or complex support demands greater structural control. Final material and design depend on case complexity, support type, drill system and manufacturing availability.

Seats on stable remaining teeth and uses reproducible dental landmarks for support. Commonly selected for single implants and partially dentate cases when seating, access and tooth stability are favourable.
Mucosa-Supported Implant Surgical Guide
Seats on the edentulous mucosa and normally requires fixation pins for stability. Denture, dual-scan or reference-scan records may be needed to align the prosthetic setup with the CBCT.
Bone-Supported Implant Surgery Guide
Seats directly on exposed bone after flap reflection. It may be considered for selected complex or fully edentulous cases where direct bony support is required by the planned workflow.
Stackable Surgical Guides voor All-on-X

Afhankelijk van de case kan de stack ondersteuning bieden bij positionering en fixatie, botreductie, geleide osteotomie en implantaatplaatsing, MUA- of cylinderpositionering en het chairside pick-up van de immediate bridge.
Als full arch surgical guide sluit het systeem aan op de uiteindelijke tandopstelling en biedt het ondersteuning voor zowel de botbehandeling als de tijdelijke restauratie binnen één gecoördineerde workflow. Niet elke case vereist elk component.
Software voorGeleide implantaatchirurgie
Forma® ondersteunt de planning van geleide implantaatchirurgie via drie beproefde softwareoplossingen. Het goedgekeurde behandelplan kan, afhankelijk van de case en de lokale workflow, worden gedeeld als PDF, samen met controleerbare 3D-bestanden in EXOCAD, zoals STL-bestanden en een compatibele 3D-viewer.

Scan and Submit (Dentist)
You upload CBCT/DICOM, upper and lower IOS/STL scans, bite registration, photographs/face scans when available, implant system and surgical-kit details. Edentulous All-on-X cases with no pre-records may also require denture, dual-scan, reference-scan records.
Design and Validate the Final Tooth (Lab)
We proposes the ideal crown or full-arch setup: tooth anatomy, occlusal plane, vertical dimension, contacts, smile position, emergence profile, restorative space, screw access and the provisional or definitive route.
Reverse Engineer Implant Placement (Lab)
The setup is aligned with the CBCT. Implant position, depth and angulation are planned within bone, soft-tissue and anatomical boundaries while considering inter-implant spacing, sleeve access and the intended abutment or restoration.
Review, Revise and Approve (Dentist) (Lab)
You review implant positions, sleeves, fixation pins, guide support and the prosthetic relationship through the supplied PDF and 3D files. Comments, revisions and approval are completed before production.
Produce and Prepare for Surgery (Dentist or Lab)
We or you (in-house) finalise the CAD CAM surgical guide and produce the selected resin, metal or stackable solution. The approved plan and relevant component information accompany the case for clinical verification; where indicated, the same data can support the immediate provisional and final restoration.
